We offer traditional and self-insurance options.
Mendez Family Care is pleased to offer an In-office Self Insurance Policy. The cost is $200 per month, payable on the first day of each month. The In-office Self Insurance Policy covers your office visit, labs that are performed in-office, prescriptions filled through our office pharmacy, and most in-office procedures.
Routine labs are drawn in office as a courtesy to our patients. The studies are then sent out to an independent laboratory for analysis. Accordingly, the laboratory that is performing the testing also charges for the work that they complete. If insurance does not cover the complete bill, the amount owed (balance) is the patient’s responsibility. Therefore, the patient will be directly billed by the laboratory. This balance owed by the patient is between you, the laboratory, and your insurance company.
Authorizations & Referrals
What’s the difference between an authorization and a referral?
A referral is a recommendation that is made by your physician or nurse clinician for you to be evaluated by a specialist. Once the referral is initiated, the referral coordinator sends the required information to your insurance company (if required by your plan) for an authorization. An authorization is the approval that is received from your insurance company for your visit with the specialist. The authorization is issued by your insurance company not by our office. Different insurance companies have different time requirements for processing the decision. With this in mind, we process all authorization requests within 72 hours. If this does not meet your needs, please contact your insurance company regarding any perceived delays.
